Tips for filling out the Proxy Form

  1. If you or anyone from your school board is unable to be present at the CESBA 2025 AGM, you may complete the online proxy form and appoint Lisa Hickman, Chair of the CESBA Board of Directors, as your proxy holder.
  2. Each Board is entitled to one vote at the CESBA Annual General Meeting.
  3. Once you have completed and submitted the proxy form, you will see a confirmation notice that your vote was received.

  4. Approve the slate of new Directors to the CESBA Board of Directors for the 2025 – 2026 year   

I hereby acknowledge that the school board I am representing is a member in good standing of CESBA and as an authorized representative, I give Lisa Hickman full authority to vote on behalf of the said school board at the September 25, 2025 Annual General Meeting of members for the Ontario Association of Adult and Continuing Education School Board Administrators (CESBA).
